Little Gate is going paperless!

All Little Gate students are required by state law to have files on record with the school nurse.

The Medical History Questionnaire and Consent for Emergency Health Care form is to be completed online by the parents.  

Please print the following form to bring with you to the physician's office at the time of your child's visit.  PDF Version of Physician Form

Verification Form

Please review and edit the verification form below for each of your children enrolled at Little Gate.  This form is only for current students enrolled at Little Gate.  Verification forms are due by May 13th.  You will use the username and password emailed to you on April 5th from publications@mcgeheeschool.com.
